It's that same stupid old assumption that you can't be "abused" until it's physical. Someone can treat a person worse than a hostage or sex slave, playing mind games that completely distort the sense of self, but as long as they don't leave a visible mark it's easy to pass off.
Mental abuse starts a long time before the physical abuse. The abuser needs to get his "victim" ready to not run when the physical starts. Mental Abuse is just the precurser.

Mental, physical .... it's like saying oil and water are different. Sure they are but they are both liquid. Abuse is wrong, doesn't matter what type it is.
